Boston University Physical Exam
- All full-time students in the Commonwealth of Massachusetts are required to provide documentation of their immunizations.
- The requirements are outlined in the “Health History and Physical Examination Report” or “Health Form,”
- The forms can be accessed on the Student Health Services website.
- In addition, extra proof of immunity must be included on the relevant “Health History and Physical Examination Report” for medical students who will treat patients at:
- Boston Medical Center
- other hospitals
- clinics
- offices
- In compliance with state and federal privacy laws, the SHS Patient Services division is committed to protecting your health information and sustaining your health care records private and safe.
- Medical records are retained twenty years following the patient’s last appointment or last treatment.
- Patients are entitled to see their medical records and to obtain copy of them
- You will need to add your:
- Immunization Documents
- Health Information
- A patient has a privilege to obtain a duplicate of their health information under both federal and state law.
- However, before the request may be processed, an adequate authorization form is needed.
- SHS has made the Medical Records Release Form available for the sake of simplicity.
- You have to:
- download the form for your own use
- sign it
- upload it online using Patient Connect.
- Please carefully follow the directions below to make sure the authorization is correct and complete, as this will aid us in processing your request.

- You must first fill out the Authorization for Medical Release Form if you were once a patient at SHS and want to obtain a copy of your medical files.
- Your request may take up to three weeks to process after we receive the form.
What is Included in a Routine Physical Examination?
During a physical examination, your doctor will check your:
- Height
- Weight
- vital signs
- heart rate
- blood pressure
- temperature
